1999 Mercury Sable white smoke hestitation

I have been pulling two lean codes from my sable. Replaced the MAF O2 sensor and fuel pump. Since the car has been hesitant and yesterday white smoke blew from the rear. It is misfiring and overheating after driving for about 10 minutes. After it is hot it takes a while for it to start which has been the problem since the onstart.

Sounds like the lean codes are the least of your problems right now. You may have a blown head gasket.

Why would the coolant not be going away into the engine and instead coming through the cap.

Also the oil looks fine as well

Because it's pumping hot combustion gases into the cooling system.

You have a break between a coolant port and the cylinder. The white smoke is burning coolant. The misfire is coolant fouling the plug and the bubbling is compression in the cooling system.
