the transmission is slipping and it is really rough shifting

i have 1999 jeep Grand Cherokee Limited addition......what is the problem an what can i do? it only does it some time it doesnt do it all the time..even if it is heated up it still does it...it does it when anywhere between 10-50 mph..im really not to sure...
