I have narrowed down the problem but don't know what part or parts I need to fix it. The metal hose going into the intake manifold is rusted and the coolant is leaking from there. Can just the hose be replaced or what will I need to replace to fix this?

Can you attach a picture of the leaking part? It sounds like the thermostat housing and/or bypass pipe, but I can't be sure without seeing it.
