1997 Land Rover Discovery Break failure

Occasionally when using my car the breaks will fail, I'm not sure what is causing this problem or where to start with fixing it either, the breaks will send a vibration back up through the peddle and makes a strange noise, I can only describe it as when you put a automatic into park before stopping, the car attempts to stop though.

Start with checking that all the pads are ok and nothing has gone metal to metal, there will be no ABS well in saying that there should not be ABS on this model so the brake system is quite basic, also check for fluid loss, start here.
