1995 Ford Explorer transmission problem?

So recently my car has started grinding. Its been doing it on and off for a few months now but it would only do it a little and then stop. Until about a week ago it was fine but now the grinding is consistent. It does it when I'm accelerating, reversing, turning. just today it doesnt want to shift out of second gear. It's so bad I can feel the car "clunking" and kind of jerking because of the grinding. I'm pretty sure its the transmission. Any tips or advice would be greatly appreciated.

have axles and wheel bearings checked if they are ok then have a trans guy look at it
