engine jerks whenever accelerating

My car jerks whenever I accelerate and come to a stop at a light, and sometimes when I step on the brake at the stoplight, the car turns off, but when I put the car in park, it starts back up. It is a '94 Prizm, and is very old...What could be the problem be?

Check to see if there are any codes stored in the computer. Next, check to make sure there are no vacuum leaks and check fuel pressure to make sure it is within spec. Check for tune up.
